Defined Fitness offers gym memberships for Workman’s Compensation Insurance Claims. To qualify, fill out an workman's compensation application at any Defined Fitness facility and submit with your medical prescription from your doctor. Please make sure your prescription contains the physical impairment and time period required for proper rehabilitation.
